INFO Riab , interview at the College of Gianni Melotti Alberto Cairo
1) The humanitarian dimension to what you heard build, and you're still building all these years in Afghanistan, and 'known to all. In short, what the numbers of this "business" and how many rehabilitation centers have opened and managed? Who works with you and how many people you got to life with your business'?
- The physical rehabilitation program of the International Committee of the Red Cross (ICRC) in Afghanistan has seven centers located in major cities across the country. Since the specific task of the ICRC to assist victims of armed conflict for several years he worked only war wounded, mostly victims of landmines. In 1994 he opened to all types of disabilities, regardless of cause. It had become absurd in fact decline as patients polio survivors, who had congenital problems or children with cerebral palsy. Wars, in addition to creating casualties, damage (often destroy) the entire health system. Without effective preventive and curative medicine, the number of disabled people is growing exponentially. Can not an intervention "vertical".
How many people have seen so far? One hundred thousand people with disabilities are those recorded to date. They come back regularly for new prostheses and physiotherapy. Every year he adds new 6:00 to 7:00 thousand (the number does not include non-disabled people, those who receive treatment after a fracture, for example). We produce 15,000 new prostheses and orthoses year, 1,500 wheelchairs and 10,000 crutches, walkers, etc. The physiotherapy are 200,000 a year, an enormous sum, but true. Since 1997 an important change occurred: the ultimate goal of the program has become the social rehabilitation of the patient. How? After rehabilitation, we help them with school, with vocational training, micro-loans and employment. We train and take a job only to ex-patients. Our employees are all people with disabilities. A form of "positive discrimination".
2) We were very impressed with a recent report, nearly 25 minutes on an Australian television. We liked the fact that, in your hearing centers are committed people with disabilities themselves. We have seen with pleasure, even a highly motivated Afghan colleagues, who was also disabled and former patient. Have you had difficulty in creating personal and Physiotherapists prepared on site? There are those who gave you a hand?
- let us just we are a team of over 600 people. It 's the team that carries out the program. Alone I could not do anything. The documentary is in the Afghan Najmuddin Helal, my right arm. At 18 he lost both legs to an anti-tank mine (a miracle he survived). Despite the serious problem, has become a physiotherapist. It manages the rehabilitation center in Kabul. A person required. Training staff in Afghanistan was not always easy. At the beginning, especially in the 90's, when we opened to all types of patients, we were with the water in the throat. We had no real physical therapists, only people with any experience with amputees of the lower limbs. Then I had to really run, teach and do the treatments. Fortunately, an American organization opened a school of physiotherapy. The level was not high, but offered a good basis on which to build. We send our students. Mandate that the disabled had caused a sensation. The head of the Ministry of Health was opposed, "poor", he said, "not make it never." Turned out the best in the course, thanks to their motivation (they had something to prove) and the fact that every day after school, came to practice. Looking back now, I think I really "press", is a demanding teacher. Some have become very good, much better than me.
3) hours Taking into account your particular profession, what would now be your "mission" of Physiotherapy in the land where you choose to exercise our profession? How did you have to develop, transform, adapt your professionalism 'the reality' where you get to work? How much do you and 'served the education received in Italy at the time? "What you have to" knowledge, know-how, know how to be "earth as a physiotherapist in Afghanistan?
- The reason we are in Afghanistan is to help disabled people get back on their feet in the true sense of the word, and then to start life anew. And I am pleased with the results we achieve, try to Afghans, the importance and effectiveness of physiotherapy. For many, doctors first, physical therapy is still a sub or a luxury western medicine, which preferred surgery and pills, often with disastrous results. To be a physiotherapist in Afghanistan must first be a solid base that allows you to deal with unusual conditions, in difficult conditions, without the help of specialists (fortunately now the Internet offers a great support) and in less time. It should be adaptable, a strong desire to learn, practical sense, ability to identify priorities. I was lucky enough to have attended a good school and, especially, have spent nearly three years in a rehabilitation center in Juba, Sudan (middle school and both belong to "Our Family-OVCI" Bosisio Parini, Como). Without the year in Juba, in Afghanistan would find it more difficult.
4) How does your team work? What 's your role and what special' medical co-operate? What are the professional relationships and hierarchies among you? How to cooperate for the successful completion of the "Rehabilitation Project Equipment" for patients who are taking charge?
- When a patient arrives at one of our centers, there are two people who receive him, a physiotherapist and a social worker. The first for evaluation and treatment plan, the other for social reintegration. The physical therapist works with orthopedic technicians, they decided the type of prosthesis or orthosis. The more complicated cases and those in need of surgery are discussed with the doctors. The multi-disciplinary approach is now the norm. At first it was easy, everyone tended to go it alone with no contact. Now everyone has understood the importance and benefits. And between physiotherapists between CPOs there are of course different degrees, depending on skills and experience.
Mi arrangiavo come potevo. Ora con internet è molto più facile. ICRC in the program I'm not the only foreign physiotherapist. Others come and stay for one or two years. Thanks to these gentlemen I have the opportunity to discuss and learn. Some time ago, for example, a talented Italian physiotherapist, Isabella Tornieri, has greatly helped to update the methods of patient assessment and treatment plan. My role in recent years it has become more and more organization and coordination. I'd rather do a full-time physiotherapist, unfortunately I have to settle for two or three hours at most.
6) What do you know of the problems confronting Rehabilitation and Italian as "echoes" about our country you come from? Have you created or maintained contact professional with Italy?
- I confess to know nothing of Italian rehabilitation purposes. I have not maintained professional contacts. I am very sorry.
7) What situations (if any) put in your crisis "knowledge, know-how, know how to be" physical therapist in your daily work? How did you learn to overcome these moments?
- What upsets me most is the daily encounter people who have become disabled because of the wickedness or carelessness of others and see my helplessness before them. Example: a boy become polio because the village head said parents illiterate that vaccination was dangerous. The new victims of landmines, weapons increasingly powerful placed where it is certain that hit innocent civilians. The porter poor ruined his back I should say to change activity, eat better and rest. He replies that home has eight mouths to feed, that's the only job he knows what to do: "I do not have a pill that straightens?" She asks, ready to flee to avoid losing time.
So, despite all the techniques and rehabilitation methods that you learn, you get the lump in my throat and do not know what to say.
8) The press called you as the Angel of Kabul or Dr. Schweitzer Italian, but as it really is Alberto Cairo, where his "dream business"? What do you still keep in the drawer? What are your expectations and, more importantly, how you live the nomination for the Nobel prize for peace, regardless of how it will end?
- When I read similar definitions laugh, what else can I do? How to define himself? One lucky person that does a job he loves among people he loves. A dream? Among the many I choose: 1. open two new rehabilitation centers (never enough). 2. To see them run by Afghan physiotherapists independently. About
Nobel, I recognize that it would be useful to talk about the scourge of shame-land mines and the many problems of disabled Afghans. But, frankly, a bonus of that given to me would be far too high. Can you imagine, after Obama, Alberto?
9) have read that in the few spare moments, read only novels, because you need to dream. Rehabilitation also read today, because now you will also come to Kabul?
- I read mostly novels, but not all. I confirm, I like to dream. Rehabilitation will read very happy today.

Queste però potrebbero lasciare sul terreno molte opere incompiute, which should then resume again with the new rulers, losing more time and energy around all'incancrenirsi of outstanding problems. Let's see some
:
- the books and orders. What seemed at hand, six months Fazio said the minister, could hardly be approved by the Senate only.
- Article 1 f if you wake up in time.
- The draft bill on freedom of occupation.
- the implementing decrees for us to enter the pharmacy.
- The equivalences of the previous titles in the implementation of the second paragraph of article four of Law 42/99.
- And many other measures that now escape me, but where we must start again from scratch.

Riab INFO , on 9 October, Health Minister Ferruccio Fazio sent to the regions of the three decrees
http://www.sanita.ilsole24ore.com/PrimoPiano/Detail/1339494 that will draw the pharmacies in the future, where will be entitled to asylum even those provided by nurses and physiotherapists. Interestingly
It is pleasing to note that, to write the decree that concerns us, has started from the 502/92, then moved to 42/99, to 251/2000 and Law 43/2006, to our profile and finally the decree on the equivalence of qualifications of the past July 27 2000, after hearing the opinion of both the 'Aifi that the Fif. As it happens, in fact, Article four of us that we can read it:
a) the definition of program performance for aspects of competence, aimed at the prevention, detection and overcoming of the need for rehabilitation;
b) therapeutic activity for the functional rehabilitation of physical disabilities, psychomotor and cognitive and visceral using manual therapies, massage therapy and occupational;
c) verification the compliance of the rehabilitation methodology implemented with the objectives of functional recovery.
All this will be payable in pharmacy to the patient's home on the order of general practitioners and paediatricians provided in Article Two of this decree. Here are some critical aspects of which it hopes to remedy in the discussion with the regions. In fact, according to what we read in this article, the benefits may be provided is the responsibility of the NHS, subject to medical prescription, always on prescription, under the private cost to the city. But the pharmacy service was created as an extension of the agreement and delivery points already as a place to practice a profession for us, however, is already without a prescription.
